What to Expect
Once you have decided to work with Gay please call or email to receive the forms to complete and return via email or fax. As soon as your forms are received we can set up a convenient time to schedule your appointment. Gay is not a direct provider for insurance so we do not accept insurance. However, with a physician’s prescription and a PPO type of insurance many patients are able to get up to four visits covered by their insurance. To know for sure you can contact your insurance company and ask ahead of time. We require payment at the time of service but will provide you with a super bill including ICD-9 codes you can file with your insurance company for reimbursement.
Patients are responsible for payment at the time of service. Payments may be made with MasterCard, Visa, American Express, check or cash. If paying by credit card, all transactions are secure and credit cards are NOT kept on file unless the patient requests they be kept for future payment.
