To schedule an online appointment, choose the service, complete the questions and credit card information, and submit the form. You will receive an email acknowledgment and the NetNutritionist forms. Your telephone or online consultation will be scheduled as soon as you complete the forms and return them for analysis.

If you are scheduling a Medical Nutrition Therapy appointment for a diagnosed medical condition and plan to file for reimbursement from your insurance company, please have your physician fax your medical diagnosis and current pertinent lab work to 214-722-1935. All information remains secure and confidential.
